What information do I need prior to treatment and why I need it?
I hold completed consultation forms and treatment records with details of your personal data and health history.
Personal data:
Address, phone / mobile numbers, email address – these are required so I can contact you for information relating to your current treatment or aftercare.
During your consultation I will ask you to consent to a preferred communication method for an appointment reminder service and aftercare recommendations.
Health related data:
NB - Treatment cannot proceed unless I collect and store these details, I will require you to consent to these statements prior to treatment.
How long will I store your data?
It is a condition of my Insurance Policy to take and retain client records for at least 7 years following the last occasion on which treatment was given. In the case of treatment to minors, records shall be kept for 7 years after their 18th birthday. Records will then be shredded or burnt.
How and where do I hold your data?
All Consultation forms and Treatment records are kept in a secure filing cabinet within my home treatment room. Only I have access to these records and I will take all appropriate steps to protect the confidentiality, integrity, availability and authenticity of your data.
NB: The personal data of Clients at Physioem Therapy is also held digitally on PC for contact reasons only. Consultation forms and Treatment records are kept in a secure filing cabinet within the clinic at 103 Main Street, Sauchie and as such clients are also required to read and sign the Physioem Therapies privacy notice.
Who will I share your data with?
I may need to share your data with authorised legal, regulatory and insurance authorities, if required to defend myself. This will be the professional membership body I am registered with and the insurance company I hold my professional indemnity insurance with. Your data will never be shared with a third party without your express permission. (i.e. other therapists or GP)
Marketing

- Consultation Form – all clients must complete a consultation form detailing past and current health issues, medical conditions, current medications and general lifestyle choices. This allows me to customise a safe and suitable treatment for you. It is also to comply with the professional code of conduct and ethics set out by my professional body and for insurance purposes.
- Treatment Records – these are my notes detailing what I have found in my assessments, what treatment I have given you and how you felt immediately after that treatment.
